When you start to leave care and approach 18, your Social Worker and Personal Adviser will discuss with you about different housing options for your future. You have told us that you would like this to be planned earlier so your Social Workers and Independent Reviewing Officers will start to discuss this from when you are 16.

Your Social Worker and Personal Adviser will update your Pathway Plan Part 1 to discuss what support they think you may need with housing and what type of accommodation would best meet your needs.
